Late night at Jack in the Box around a year ago. Just completely gave me the wrong order. I amazingly was ok with everything they gave me anyway so just kept it and ate it. Paid in cash, btw.

Many years ago there was a Wendy's "W Burger" combo that I ordered via drive thru. The burger was actually a hamburger so I went inside with receipt to fix it. Since it was during the lunch hour I waited a while in line to show it to the cashier who then showed the manager who then visibly berated the staff in front of me while apologizing and correcting my order. Interesting experience, would do again.
