Accident in a virology lab in China. From what I read they were conducting tests on a new strain of coronavirus in the lab and one of the assistants got splashed with bat blood. That got them infected and they died. However no one who was handling their body was notified of what they died of and contracted the disease. That person then spread it to others.

Also to point out, that lab was actually not far from the wet market that people say this all originated from

Not world governments as a whole. China's yes, the US yes, but some countries handled the outbreak amazingly
