There's no reason to expect a person to go through a game more than three times to receive a proper ending. That's not about challenge. That's not about paying dues or putting forth the necessary effort. If there is no new content, there is no reason why a fourth play-through should be essential to getting the true ending.

A game's length should be directly tied to its variety. If it only has seven levels, it should max out at 20 to 30 hours of gameplay before one can say they've beaten it. You shouldn't have to repeat the same tasks over and over and over and over and over and over and over and over and over and over and over and over and over and over and over and over in an artificially elongated manner to prove that you have the willpower to achieve the "true" ending.

If you've beaten the game on hard mode, you've beaten the game. Fuck any programmer who says otherwise.

there is also no reason to lock shit behind hardest difficulties and rankings. eg dmc.

the nuts that want to go for S rank will still do it without the unlocks that are ties to it.

all it does is fuck over everyone else.

I don't begrudge having unlockable difficulty settings or or difficulty settings higher than "hard." But you should absolutely be able to see the true ending if you've beaten the game on hard mode. Those should be optional for people who want to return to the game at a later time and re-experience the initial challenge of the game.
