Even as freakish and demented as he's portrayed, he could still manage to attract certain women (including those convinced they share interests...as also demonstrated in the comics). So it's not really a case of the Joker being incapable of finding women willing to be with him. It's more that he's a psychotic megalomaniac mass murderer who's already got a passion that satisfyingly sustains him and finding a girlfriend is understandably depicted as a lower priority. Could he get a girlfriend? Surprisingly yes. When and why would he bother? That depends on the writers... Harley Quinn can be considered a kindred spirit to the Joker, but I've always seen calling them a couple as an overestimation. As he is typically written, the Joker's not really strongly committed to anyone but himself. Harley is generally just a convenient cheerleader in his schemes. It's kind of symbolic how the Joker has dubbed himself royalty in the past while Harley continued playing the bit part of his jester...not a queen. It would be more accurate to label Joker as an open misogynist since his track record of treating women includes a horrific history. Jack Napier was a rich gangster before he became The Joker. I think he had some ladies around before and after. The Dark Knight Joker mentions a wife in one of his origin stories. Although he's probably lying I don't think this Joker has issues with women at all. He just had fun twisting the morality of the public and the people around him. He isn't nervous around Rachel and doesn't get butthurt when she rejects him. Suicide Squad Joker has Harley. As does Animated Series Joker (who generally has the Jack Napier origin anyway).
