Yeah for me it's mainly the situation and how they deal with it. Serious and dramatic, but not overly grotesque.

Personally I feel gore can be overrated on the fucked up scale. Except for the elevator scene in Made in Abyss

I loved it, never read Salem's Lot but I loved the general horror theme of the anime. Wish I could find more like it.

It wasnt the violence with devilman crybaby for me. Maybe I interpreted the scene wrong but I recall one episode where the MC more or less raped a bad guy. Sure she was trying to seduce him at first but Idk if she was still interested after he hit her. Also iirc he was beating the fuck out of her while fucking her.

While I wouldn't consider the anime adaptations to be particularly fucked up; the Berserk manga is the most fucked up fictional universe that I can think of off the top of my head.

If women aren't getting raped by trolls so that troll babies can eat their way out of their wombs; Guts is using the blood from dozens of children transformed into rapist fairy demons in order to protect himself from fire. The Lost Children arc is so fucked up, no video game or anime adaptation of Berserk wants to touch it.

The redeeming feature being that almost all of the fucked up people in Berserk usually get what's coming to them.

To be fair, I kind of cheated with the last one. I don't think they ever adapted it into the anime for obvious reasons, but it's in the manga.

Really, in terms of tone, the first one is hilarious, the second one's trying to be edgy and failing, and the last one's genuinely disturbing.

That's how I feel about anime as a whole compared to manga. I'm pretty sure I've seen most of the stuff listed ITT, but everything happens so fast and it never really sinks in. Meanwhile there's shit I read that's way less graphic, but manages to stick with me much harder and leaves me feeling genuinely uncomfortable. Shit like Goodnight Punpun fucked with way more than anything I saw in Blood-C or Higurashi. Idk why. It's weird. I guess it's because you tend to spend more time reading than watching so you're a bit more invested.

On topic, most things have been listed already. I guess Paranoia Agent is worth a mention.

it's been a long while, but iirc, there was a cult who wanted to bring forth an anti-Christ like figure, and the leader of the cult raped his daughter which led her to being impregnated with the dark lord.

the dark lord grew in her womb until he was full grown and ripped his was out of the young woman.

there was other messed up stuff going on, but that's what I remember most

and I only really watched it because I thought that it was related to Ninja Scroll.
