Huge metropolitan areas in the game are instead broken up into a series of towns, villages, and cities of various sizes, with the bigger ones generally only accessible through guard buildings. This implies that in this world, pokmon are so dangerous that cities have to be walled-off and heavily guarded to keep the citizens safe. It also implies that the smaller towns, which don't have such protection, are small because they don't have protection.

The small towns don't need protection because the pokemon outside tend to be low level junk monsters, and the cities have big strong gym leaders that can push back any existent threat

Though WTF Sabrina was doing when Team Rocket took over Saffron kind of throws that theory out the window.
