It doesn't ~~usually~~, just means that they're not going to pick a side to agree with everything on - in some cases that may indeed mean a middle ground, but it doesn't have to. The perfect example - if Person A says "let's kill 1000 cats" and Person B says "let's not kill any cats", the stereotype of a centrist would say "let's kill 500 cats", while an actual centrist would most likely say "let's go with Person B's idea here, but I might agree with Person A on other issues". For example, I'm in favor of progressive tax rates, universal healthcare for citizens, increased funding for public services and infrastructure, union protections and benefits, pro-gay marriage, some firearm regulations, etc. But I'm also in favor of maintaining a strong military force, mandatory military service for all able-bodied citizens, a strong police force, strict border controls, very low immigration and refugee admittance, and the right for citizens to own and carry firearms. I also have a moderate nationalist disposition, in that I prioritize the prosperity and well being of the US and its citizens and its interests even at the expense of other nations. And so on.
