I thought the office of mayor could only exist at the municipal level but somehow Kane is mayor of a Tennessee county.

What part of the country are you from? I'm in the Northeast and counties are pretty much just used for statistical purpose here, but I know in a lot of areas (especially rural parts of the country), counties have pretty significant governments.

Where I live the county seat is administered by county commission thats broken up into five (I think) districts but I live in Florida.

Same here in Nebraska. Most of the counties have 5 commissioners, except Douglas County (containing Omaha) which has 7, and some of the tiny counties like Arthur (population 460) which has only 3.

Fun fact: The smallest county in the US, both in population and land area, is Kalawao County, Hawaii. It's only 12 square miles (31 sq km), and has a population of just 88 people. Donald Trump finished third in the 2016 election in Kalawao County with just 5% of the vote, behind Hillary Clinton (70%) and Jill Stein (25%).

I can only assume that some areas aren't populated enough for conventional government so they have to break it up. I've been to some places that pool resources (including some government officials) although I can't recall it being a whole county. Otherwise "county" might just a name rather than a traditional county.
