Basically use arx fatalis as a template set in fallout universe, maybe not long after the war, in a vault where shit started going wrong and the MC try to escape the underground experiment.

The gameplay is like well, arx fatalis, or even system shock/prey / bioshock where you try to escape from mad overseer and minor factions formed against, or supporting the main antagonist. It will feature non linear gameplay where you can solve problems multiple ways, etc.
