One of the biggest pieces of advice I can give you is this:

-Make sure you play through the main story until you do the quest "Train Hard, Fight Easy". This is when you learn how to do things like counterattacks, perfect blocks and dodges (and some other combat techniques). Combat can already be fairly difficult in the game, but without these techniques it's much harder. A lot of players tend to go off exploring before getting to this mission (myself included when I first played it) and after doing it combat became much more manageable.

Other than that here are a few other smaller tips:

-You need to eat and sleep regularly in the game, so you should always keep some food on hand. Food also spoils after a certain amount of time (past a certain point it can actually physically harm you).

-People can judge you based on appearance. If you get dirty from exploring and combat, and/or wear cheaper looking clothing/armor higher class citizens and merchants may not treat you as well. At the very least try to make a habit of cleaning up at any troughs you find (these clean you up somewhat, if you want a full clean you need to hit up a bathhouse).

-Learn to read as soon as you can (I can't remember the name of the town, but there's a scribe who will teach you). This will allow you to not only read certain books/documents related to certain story/side quests, but you'll also be able to read skill books that can help boost your skills.

-Assuming you don't mod the game to allow for saving any time make sure you always have at least one or two Saviour Schnapps on you at all times so you can save if need be. Some of the autosaves are fairly wide apart.

I thought a patch made it so you could save and exit at any time, now?

At the beginning you need some decent gear. An easy way to get it:

Sometimes when wandering around, a random knight will challenge you to a duel. It's generally outside of a city. If no one else is around, refuse, then sneak behind him, knock his ass out, and steal his equipment! This is the best way to get good gear early on.

You can save when you exit, but from what I remember it was designed only so you can save in the event you needed to quit the game. Once you loaded up that save the file was then deleted. So you couldn't use it like a traditional save and/or spam saves.

If you're on PC I'm sure there are mods to enable saving anytime without Saviour Schnapps though. I just felt I didn't need that once I finally started doing alchemy and making my own potions.

How does this game run? I really hate stuttery games (almost all Bethesda now) and they piss me off.

Partly the reason I'll only play Witcher 3 from now on.
