He would probably have to get a boner in order for it to look right. It would also be difficult because he would have to maintain that same boner strength throughout the process.

I doubt dick tattoos happen often enough for there to be a standard protocol.

You'd be surprised. I worked nights at one shop for 6 months and in that time we had about 100 people approach us for them. I'd say about 20-25 of them were serious and sober at the time and ready to go right there. The rest were drunk, and thus illegal to tattoo....not that it mattered as no one at that shop would do them.
