Only two options really. I suppose people who don't buy new systems often wouldn't have one ever break but that's boring.

Synchro summons were the start of the decline in quality. With pendulum and xyz being the finishing blow. Keep DL vanilla, keep DL good.

I had a 360 RRoD and my SNES just stopped turning on one day. I also had three lemon PS4s, which was unusual. The first one had a faulty HDMI port, the second wouldn't play certain games for whatever reason (Hotline Miami and Gat out of Hell were the two I remember), and the third would lose video/sound/network randomly. They had these issues right from the start, so it was bad luck. My fourth was the Destiny Taken King one and it has been fine so far (and hopefully continues to be).

Lucky i guess. Only had an N64 fail due to a preventable power surge and my launch 360 which is apparently what people expect to fail most still yet lives :o
