Depends how much I like/hate the job and how long I plan on staying there. When I was younger I had a part time job that I hated and that had pretty similar hours to your guy's; 4am to 1pm. If they told me I would need to come in earlier because someone else was taking off, I'd just say no sorry, I can't do that.

If it was something I liked and wanted to do for a while, I'd bite the bullet a few times but if it became a regular thing, I'd probably stop agreeing to it.

And you said no right? I mean I've never heard a case where someone got fired because they didn't do overtime. The issue is more with the manager that keeps approving your coworker the time off. The year just started so I'm not sure how USPS works their PTO, but eventually this guy will use all of his.

Considering you don't really know that person's medical history, and sick days exist, it shouldn't really affect you that much. Besides, it's not like you've been denied the courtesy of doing something like that, and if you really wanna be petty you can always use it as leverage down the line when you want a day off.

Your boss authorized his day off. Deal with it. His loss if he wants to use more sick time off for something that shouldn't take his whole shift. And you'll be glad to be able to take the whole day when you're the one who needs an appointment.
