I don't think he understands, or is deliberately trying to redefine, the word elitist. An elite surgeon is not the same as an elitist surgeon. The former, yes, that's what is needed. The latter is probably dangerous.

Meritocracy would be a better term for what he wants in our western society. Calling someone an elitist is not necessarily denigrating their expertise, but an accusation that they have contempt for common people. You can express views from a place of experience and competence without also expressing contempt and hatred of the "lower life forms"
