IIRC it refers to the acidity of the olives. Olives become more acid the more time they spend off the vine, so "extra" means the oil was pressed out almost immediately. Virgin refers to it being the first press.

I may not be 100% on this, but this is what I remembered from when I was in Italy

I know for what the vast majority of people use olive oil for, buying extra virgin is a waste of money. And chances are what most people buy isn't really extra virgin anyway.

basically it's this.

The process is something like.

grab olives -> Mash / Get oil -> That's extra Virgin Olive Oil

Add some stuff to the mash (usually lye) -> Mash a second time -> Olive Oil (or Virgin Olive Oil)

Repeat again -> Extra Light Olive Oil

the difference is predominantly in how much olive flavor is imparted into the oil. Extra Virgin Olive Oil has a strong taste while Extra Light Olive Oil has a weak taste. You'd use one or the other based on what cooking goal you were trying to achieve.

Maybe you haven't noticed. It definitely exists. I use regular olive oil for cooking, extra virgin for seasoning/marinade/stuff requiring flavor.
