Died only once at Shadow Link, in probably some of my sloppiest fighting ever.

Even better I got it all on video.

I have done a perfect 000 clear once before.

This is a bit different than other Zeldas.

It is no save, any save counts toward the counter.
However you have extra lives. As long as you do not run out completely, (use a Continue) then you can still achieve a 000.

I had one death at the very end of the game.

I actually cast Shield for once when I was losing to Shadow Link, but to no avail.

Also, playing on console and CRT television.

A few restrictions I impose on myself:

-No using the Shield Spell at all (did it this once out of desperation, you'll see I only cast it near the end of the fight

-No Reflect spell....that's no protection against Doomhammers hammers or Thunderbirds fireballs.

Wow, and I remember not being able to beat it at like 14 without putting restrictions on myself. I gave up too quickly though.

Yeah except I cast Shield this one time because it was too near the end of the game. But to no avail.
