Usually they were bundled with the game new, but if you bought a used game and the Online Pass was used you'd have to pay $10 or something to access any online features like multiplayer or co-op.

Of yeah that! Haven't bought many games that had those. (haven't bought non-Steam games in a while so that might be why) but I recall a few 360 games having those, bonus stages and costumes mostly from the ones I recall, but others had DLCs in them.
