Well, by definition, it refers to people who are attracted to two genders. Another letter could easily be added for a word representing people attracted to whatever other genders they believe exist. I don't believe the "Q" was always a part of the acronym.

To further elaborate, bisexual usually means "attracted to cisgender female and male individuals only." People who are attracted to cisgender and transgender female and male individuals would be considered pansexual.

I know that sounds weird, but even if you like both sexes, you might not be attracted to someone who's transgender.
