I was only watching it on-and-off, but I think the main problem came out of the Wind Temple, causing a chain reaction.

IIRC he messed up something in Wind Temple, which messed up his Forest Water timer. As a result, he couldn't do everything he wanted to do after Wind Temple but before Cabana Skip. However, since he's bound to the Forest Water timer he had no choice but to just absorb the remaining time despite doing less. So, not only did that take up time, but he was also was down a chart.

Later, I believe he got trolled by Puppet Ganon doing something Cosmo had never seen it do before, which cost like a minute. There may have been other stuff in the middle too, but as I said, I only watched a few parts of that run.
