The implementation varies depending on the game, but generally they are random packs of gear/equipment/"loot" that players can purchase for in-game credits and/or money.

Well for a specific example, in Infinite Warfare, you can earn keys from playing games in multiplayer or zombies. Game mode, length of game, score etc all play a part in how many keys you earn for that match. You can then take those keys to the armory and buy common or rare loot boxes for 10 or 30 keys respectively. They contain things like camos for guns, emblems, calling cards, outfit pieces, gun accessories, guns themselves, gestures and so on

Edit: I guess what usually makes people mad about the whole thing is that you can also pay real money to open the boxes, so since you can get guns from it people say it's pay 2 win. I actually don't really mind it as it gives more reason to keep playing, something to still work for
