Like, if you see no mold on the surface and you cut the bread in half, you'll never see any in the middle?

The spores come from the air so it makes sense that the mold is gonna form on the exterior

The mold will spread from the initial point of contamination.

Baking bread -typically- renders it sterile. Any contamination after it comes out of the oven will naturally be on the outer surface.
