i.e. Volgin from MGS3, every Fire Emblem villain, Gaius from Tales of Xillia, Emperir Palpatine, etc. Why can't we get a 70 year-old dude with an amputated leg who still has massive control over the kingdom/empire due to other things?

Well, that's easy. People like the idea of the leader being the final challenge to overcome and not one of his subordinates. The leader being taken down easier than the subordinate is usually seen as anti-climatic.
