Hell they even gave you dilagoue options to make it seem like noct didn't give a shit about the romance but he suddenly greatly cares at a specific story point.

Not gonna go into details but I feel it was an arranged marriage and that's why you where given options to act indifferent about it. As for why he acted like he cared all of a sudden despite if you chose to act indifferent I just chose to feel like it was because the situation made him feel powerless to protect a single individual let alone his country.
The biggest problem I had with the game, at least with the romance story, is they hinted towards Noctis and Iris being something, and then poof... gone.
I didn't play the game after the first couple months so I don't know if they brought her back in some sort of DLC, but her disappearing after the time gap just felt weird.
