That is something I wish they would have at least touched on. I don't remember the specifics, but I got the impression ghouls were made from experimentation/drug cocktails, but the last episode made it seem like it was to do with the nukes.

Again, never played or really know much at all about the games. Doesn't change anything not knowing, but Mama Ghoul kinda made me go 'wait, how?'

This is the one thing the show does actually retcon with no explanation. Ghouls are indeed created by radiation. That's why Cooper tells Lucy he's going to turn into him when she drinks the irradiated water.

The drug they take is new to this show and breaks a good number of things about ghouls in the games.
