There are cassette tapes which work like ink ribbons in the older games. There are a finite amount of them and no/few respawn points.
Items are moved around and changed.
Enemies aren't just stronger, they're a little faster.
Jack begins hunting you earlier.
There are more antique coins to collect and more cages to unlock. They put the Scorpion Key in one of the cages, a coin that you hold in your inventory to increase your attack, and another coin you hold to increase defense.
To add to this:

Enemies aren't just stronger and faster. They're also more aggressive and some of the placement has been changed so people on a 2nd playthrough are still taken off guard.
Jack also hunts you for longer and more aggressively.

It's more than the cliche shit game design "Hard mode just makes you do less damage the enemy do more damage" they actually put thought into it
