Yeah, there's risk, but such is the nature of any medical treatment. Whether or not the potential therapeutic benefit is worth the risk is a decision that needs to be made between the patient and their doctor (including a therapist, in cases where dysphoria/dysmorphia is involved). Unless you've got concerns that the doctor has inadequately informed the patient of the risk and/or is pushing the treatment against the patient's best interests (which can be valid, particularly with lucrative cosmetic procedures), that's really the beginning and end of it.

And, again, this is a question of "your genes dictate how you're meant to be" being treated as a valid criticism. Expressing concerns about safety is very different from trying to insist that somebody is inherently wrong for wanting to change.
