Very effective at preventing further loss in androgenic alopecia, and I think the figure is that two thirds of users will have grown a significant amount of new hair following one to two years of use. I've been on finasteride for a little over a year and a half. My hairline hasn't receded any further, and the thinning that led me to treatment is much better.

I think the risk of side effects, and stories about them, tend to be overblown, but if you have issues, pyrilutamide is a promising treatment to deal with DHT by a different mechanism of action and it may be a year or two out. I'll look into stacking it with finasteride when it becomes available.

I've just hopped on topical minoxidil in the last couple of weeks with the hope that it will promote regrowth along my hairline and temples. I'll give it a year or two. edit: Actually, I'll be on it for life, because by that point some of my hair will be dependent on minoxidil. I just mean I'll give it a year or two before I decide whether I want to get even more aggressive with my treatment.

re: transplants, keep in mind they aren't an alternative. Most doctors will refuse to perform the procedure if you aren't already blocking DHT.
