For the record, I think you did fine in your message unlike what some previous posts have said. Only thing I'd say is you didn't need to mention her bad response time.

Pretty much just say hey I've tried to meet with you several times and nothing's working out. Good luck to you.

I mentioned the lagging responses because we used to have lengthy conversations each day, which turned into a single reply every 2-3 days. Obvious sign that Im not a priority, so I let her know the message was loud and clear.
