If a ride allows operator error to kill people, that's a ride error. Especially when "operator" doesn't mean a skilled engineer who's been extensively trained in the safe operation of the device, it means some teenager getting paid minimum wage after being trained by a slightly older teenager making slightly more than minimum wage. That means operator errors are pretty inevitable and need to be planned for.
A ride error is when the ride has either been maintained poorly, is very old, or is poorly designed in some way. An operator error means that the operator screwed up, or made an earnest mistake. A guest error is when the guest makes a stupid decision that causes them injury or death.
