I think the only controllers I've ever had it happen with are 360 controllers.

Most of the time, if a controller dies on me and needs to be replaced, it's because one of the A/B/X/Y (X/O//) buttons goes wonky.

Or because I'm playing a particularly hard game, and it ended with me flinging my controller across the room so hard it exploded (though I haven't really done that since the PS2 era).
