States are allowed to discriminate for non-prohibited reasons against non-protected classes. For example, states could enter registered voters into a lottery and randomly select 10% of them to be eligible to vote. There may possibly be a right to autonomy in making healthcare decisions for oneself, although it isn't expressly stated in the Constitution.
