Another person was killed when a metal cleat on the hull of the Columbia tore loose, striking a guest in the head. The normal inelastic hemp tie line had been replaced by a cheaper nylon rope which could stretch. The ship approached the dock too quickly and and an employee slipped the rope onto the cleat to brake the ship, something it was not designed for. The rope stretched under the load, ripping the cleat free of the wooden hull and launching into the crowd at high speed.

One person was killed on Big Thunder Mountain Railroad and ten others were injured when the train derailed. The locomotive fell on top of the first car, crushing it.
These are the only ones that weren't caused by guest behavior.

A cast member was crushed to death by the America Sings ride, caught between a stationary platform and a revolving wall.
According to other cast members, she used to play chicken with the revolving wall, not a good idea
