Guyana and Suriname are like Caribbean nations. It's rare that they can even qualify for the Gold Cup. They would just be annihilated and have to pay way more in travel costs to be part of CONMEBOL. They are best served being part of the Caribbean Union (which is part of CONCACAF). French Guiana is part of France and I don't think CONMEBOL has any interest in having something like that. I don't know how many confederations allow as many non-FIFA teams as CONCACAF due to the CFU. Thank colonialism I guess.

As far as Qatar - both CONMEBOL and CONCACAF have guest nations in their tournaments. Qatar has recently played Copa. USA has played in the Copa. Heck USA even hosted Copa*

maybe the short answer is "outside of Mexico everyone in CONCACAF isn't that great at soccer so they all stick together, and USA will host tournaments for them if we get to play against them"
