If 100 people are vaccinated, perhaps 90% of them become immune.

That doesn't mean the other 10% will catch the virus. They might not be immune, but they might protect themself via social distancing / etc, they might get lucky and never come in contact with it, etc. In turn, this means some of the 90% might also be in this situation - even if they are immune, they never encountered the virus anyway so it wouldn't have mattered.

They need to adjust for these factors when running the trials; which is why they compare how many vaccinated vs unvaccinated people caught the virus, not just "how many vaccinated people still got sick"?
