The nasal spray contains a live, weakened (attenuated) virus that cannot survive or replicate at internal body temperatures. This is a strain of flu that does not normally cause disease in humans (though there may be risk for those with immune deficiency). They take this non-pathogenic virus, and add in the surface antigens of the strains thought to be widely circulating. This allows you to mount an immune defense against the "bad" flu, without ever having to be infected by it.
