Everything is a team effort and if something was overlooked then the whole team missed something because they all worked on it at some point. Plus, pointing blame is never going to solve anything, you should instead work on finding root causes, then implementing safeguards against them to make sure that the same problem never happens again. --- I am a Contract Software Developer.
