To get maximum points, you actually have to hook at least 5 times, which means someone was saved at least once. Both do in fact apply. Also, as long as you don't go especially far away, in the time it takes you to get back after the unhook notification you should usually be able to find either the original victim or the person that unhooked them, assuming you didn't just find someone else in the meantime.
