They sorted that out in a comic book. It was basically never spoken of in the games again after Ac3 outside of a few mentions here and there.

That's something that Mass Effect got really bad about.

Jacob and Miranda were from a phone game, though ME2 is a strong enough product that their introduction is fine whether you know about their origins or not. But their friend Ish is also from that game, and he sticks out like a sore thumb - it's some real Remember The New Guy shit. Then there's the Shadow Broker DLC with Feron - he's a flat character, and his chemistry with Liara is barely there. The main appeal of Feron is to those who read the comic book about Liara obtaining Shepard's corpse.

But things really went into overdrive with ME3.

Emily Wong: killed off-screen during a viral marketing campaign
The Illusive Man: Backstory explained in a comic book (which I just learned now using Google Fu)
Kai Leng: Book villain, worst character in ME3

The 7th generation of video games were lousy with video game novels. Kind of a shitty period imo - it's interesting in how video games tried to make that leap into becoming big multimedia properties, but it all went to shit when game studios started folding external lore into the core video games.
