Being an online game also inherently prevents community modders from creating effective patches, which is how most previous Bethesda games were made playable.

When Skyrim is a buggy mess and a week later four people come out with unofficial patches that fix 90% of the problems, most people are less likely to complain.

When Fallout 76 is a buggy mess but those same modders can't fix it at all due to anti-cheating blocks and server-side processing, you're left waiting for Bethesda to fix it, and Bethesda never fixes anything.

If anything, Fallout 76 is the direct result of their previous games. After years of having the community constantly fix all their mistakes, they developed a very lax attitude towards bugs, because they were never really a problem in the past when other people would fix them. But Fallout 76 was the first time they couldn't rely on that particular safety net, and they wound up crashing and burning because of it.
