The problem is infrastructure. You say some nations have poor distribution, but a possibly even bigger issue is how difficult it is to ship to many areas. If you don't have decent roads, highways, railways, ports, etc., it becomes very difficult and expensive to get the food to the people who need it.

This is really the bottleneck. There are so many countries in Africa that have muddy trails like the one in the picture I will link below instead of actual highways that cross their countries. Usually you will have some paved roads in and around the capital city and maybe a few other big cities, but they don't extend out into the countryside in many African countries.

That's supposed to be a national highway, and in the second picture you see people having to use shovels to try to dig their vehicle out of a position it got stuck in. It gets even worse in the rainy season when the whole thing turns to mud.
