If it's "Congratulations on your wedding Todd and Howard" with two groom figures, I don't think there's any way you could say the person is being forced to make a design against their wishes unless they would refuse to put the names of a straight couple and groom and bride figures. I feel you'd need an extremely flamboyant cake to have any grounds for refusal. He can refuse to do any customization he wants, for any reason he wants. As long as he offers a product of reasonable equivalence. Hell, there's nothing wrong with refusing to write Steve on a cake because you don't like the name Steve.
