If it's "Congratulations on your wedding Todd and Howard" with two groom figures, I don't think there's any way you could say the person is being forced to make a design against their wishes unless they would refuse to put the names of a straight couple and groom and bride figures. I feel you'd need an extremely flamboyant cake to have any grounds for refusal. You can literally refuse to make a white cake if you're so inclined.
