There most likely is a safety net. I can't imagine that breaking a vial would be so uncommon that there wouldn't be a procedure for getting it replaced. The TC likely just didn't follow it.

TC, why would you just have one vial? Why wouldn't you have a backup precisely in case something like this happens? Why wouldn't you have refills? Do you literally wait until the last minute to get a refill? A lot of that sounds like it was preventable, and you just didn't do it.

I am on blood pressure medicine, and as soon as my refill counter hits zero, I call up my doctor to get more refills. I don't wait until I have one pill left to get it refilled.
