As genocet (very surprisingly) said, pharmaceutical addictions are usually the gateway to heroin. People also clean up and then relapse, taking their normal dose, which kills them. I know a user here posted a story about a friend doing this, once. Many people associate shooting it with death. They think that smoking it is not fatal, etc. The addicts you will see in substance abuse classes look like everybody.
