If it's not actually in the document you're not altering it thus it wouldn't be called an amendment in the first place. And the amendment specifically requires the VP's sign-off that the President is unable to serve. The entire cabinet could unanimously declare a President unable to serve but if the VP refused to agree it would not take effect.
