I had a conversation with him when I first found out about the mistake, and then I had to sit down with him and a witness and do the official "this is what you did wrong,these are the consequences, sign this please". Yes. There is the initial paperwork of the write up that I, the witness, and the employee have to sign, that gets sent off to corporate and filed.
