This is as much a fuckup on sony's part as it was CDPR's The internal QA basically checks the game doesn't brick the PS4 and hits standard features like the OS still working. It does not check if the game has issues loading textures or has low framerate. Keep in mind the game was delayed from November because it still likely didn't hit those standards somewhere.
