The problem is there is almost no way to know that by looking at just anyone on an observation.
You can make plenty of deductions and what not simply by observing.

Look at a any healthy person. Ask questions like what is a healthy person? What do they eat? How long do they sleep? What do they do for exercise? What do they do as hobbies? What do they shop for at the grocery store.

You can easily compare yourself and your lifestyle to that of a healthy person and a healthy lifestyle, weed out the bad habits and adopt good habits. But doing that requires you to shift your mentality from negative thoughts and ideas such as "I can't do it because it's hard" or "I'll never maintain it for long", to positive experiences and thoughts like "I can do it because it's worth it" or "I can maintain it and improve bit by bit every day.

Your mental state is a reflection of your physical state. Fix your body by fixing your mind. If you wanna go one step further, you can fix your spirit/soul by fixing your mind. If your mind is fucked up, your body will be fucked up because everything your body interacts with on this 3rd dimensional plane of reality is a reflection of your fucked up mind, whether it's the people you are with, the place you live, the place you work, the food you eat, the hobbies you do, etc.
