It is already incredibly difficult to transition and that does a good job of weeding out almost all false cases by the time when hormones or surgery are even on the table. "There still might be some that regret it or weren't trans!" isn't really an argument. Surgeries kill people. Drugs kill people. Bad doctors exist. As long as they don't ruin the vast majority of people, then the risks are acceptable.
