If total kills exceed 50% of the population seen, the city is irrevocably thrown into chaos, and the very high chaos ending is depicted. However, these thresholds are not absolute, as there are several other factors that increase or decrease chaos. The other factors being sidequests and such. It's pretty obvious what raises and lowers chaos in those situations. You complete the sidequest where you poison a bunch of people, chaos increases.
